Yesterday's transit meltdown at Grand Central was caused by "human error," the MTA reported, after someone at Metro-North decided evening rush hour would be a good time to disconnect half the power supply to the railway's main control computers, for maintenance. A loose wire then disabled the other half.

Witnesses Describe Gruesome Aftermath of Deadly New York Train Crash

In what one firefighter described to the New York Post as a "bloodbath," three of the victims' bodies were ejected from the train, which was traveling at excessive speeds as it rounded a bend under the Henry Hudson Bridge in the Bronx.

A senior city official told the New York Times thatthe operator of the Metro-North train that derailed today, killing four passengers and injuring scores more, told emergency medical workers that the crash occurred because the train's brakes failed. The Times notes that the operator's account has not been confirmed.
